New Client Information

Welcome to the New Client Information page.

Here you will find information on our basic procedures for getting started at Kenwood and the paperwork involved.

How do I get started as a new client?

Those interested in setting up an appointment at Kenwood should call

612-377-9190, ext. 1. Here you will go through a Confidential Phone Intake that includes the following information:

  • Name
  • Date of Birth
  • Contact Information (Phone number, address, e-mail if applicable)
  • Any Applicable Insurance Information (Name, ID and group number)
  • A brief description of any presenting problems or concerns

An intake coordinator (Jane, Julie, or Joey) will use this information and any additional requests to match clients with a counselor. They will do their best to set up your first appointment with a clinician who can best fit your needs.

How long will it take?

We do our best to schedule clients up with their first appointment within the first phone call. In addition, counselors will be available within the next week unless otherwise stated up front. Some counselors may be available as soon as the next day.

What should I bring to my first session?

Your insurance card, information and completed paperwork (which can be found below) will dramatically speed up the intake process. If you do not complete the paperwork before your first session, please arrive 15 minutes early for first appointment to do so.

Attached below is the paperwork booklet that all new clients will need to complete. You may print it out and complete it before your first session to expedite the process if you so choose:

The New Client Packet

Below is that same New Client Packet, divided up into sections for your convenience. You may download one or all of them depending on what you need (Note: You should read through/complete all forms prior to your first appointment if at all possible).
